Pulmonary vein isolation (PVI) is the preferred ablation strategy for patients with paroxysmal atrial fibrillation (AF). However, non-paroxysmal AF (NPAF), there no consensus on most efficient (AS). We aimed to compare efficacy and safety of different strategies Atrial Fibrillation . performed a comprehensive systematic search using four large databases include randomized clinical trials (RCT) evaluating AS in NPAF. excluded studies <100 participants. defined endpoint as recurrence tachyarrhythmia (AT) >30 seconds after catheter during minimum follow-up time 6 months (or longest reported). estimated log risk ratios (log RRs) random effects network meta-analysis (NMA). Surface under cumulative ranking curve (SUCRA) was used rank CA (higher suggesting higher efficacy). Safety occurence one above: femoral vascular access complication, pericarditis, stroke/transitory ischemic attack/arterial embolism, pulmonary stenosis, cardiac tamponade/hemopericardium/effusion, atrioesophageal fistula, septicemia, internal bleeding, phrenic nerve palsy, death. included data from 32 eligible RCTs, total 6950 participants, mean age 59.4±8.2 69.3% male, 15 arms our NMA (Figure 1a). PVI concomitant posterior wall (PWI) associated reduced incidence AT RR 0.67( 0.47; 0.96) compared alone. Otherwise, significant difference rate recurrence. The top 3 rankings based SUCRA were hybrid ablation, ethanol followed by PWI. There low prevalence outcome- 281 (4.1%), driven predominantly complications - 243 (86%).There differences between strategies. Our analysis suggests PWI linked decreased PVI.
